To book your child onto the Stewart Fleming After School Club please read our Terms & Conditions and complete the application form below.  

The form should take around 2 minutes to complete and will only need to be completed once per academic year (IF YOUR INFORMATION CHANGES YOU MUST LET US KNOW).
